Client Pre-screening Form

Dear EURONAILS Client,

​As we transition back to operating our salon, we ask all of our current and new clients to familiarize themselves with our new policies as they relate to health, personal hygiene, sanitization protocols and prevention of pandemics as required by local government and health authority. EURONAILS is committed to health and safety of all of our clients and staff and as such we have implemented the following protocols to ensure that our services can be performed in a safe environment and in compliance with health authorities.

Please read our updated policies carefully:

1.  All clients must report if they are experiencing flu-like or COVID-19 symptoms prior to their appointment and must NOT enter the salon's facility if exhibiting such symptoms. In the event of flu-like and/or COVID-19 symptoms, appointment will be cancelled and re-scheduled until full recovery (and if applicable testing has been completed and symptoms cleared).

2. All clients must abide by posted physical distancing protocols while in our facilities. 
*This includes avoiding coming to the scheduled service earlier than 5 minutes or staying in the salon past the completion of the service. 
**Salon facilities are for clients only. Please note that strict occupancy rules are in effect and there will be no seating in the waiting area. Accompanying persons, children and pets are not permitted in the salon.

3. All clients must wear a mask.

4. All clients must follow hand-washing guidelines.

By agreeing, you confirm to that you understand and will follow our policies as described above. 


By disagreeing, you confirm that you understand that EURONAILS management/staff have the right to refuse admittance/service at EURONAILS facilities.

In the past 14 days have you been in contact with anyone who may have COVID-19 or similar symptoms?
In the past 14 days have you travelled outside of BC and have to be on self-isolation?
Please measure your current body temperature and confirm if it exceeds 37C.

Please note that if you have answered "Yes" to the above questions, you understand that you will not be permitted to enter EURONAILS salon until you have been cleared by a medical professional. We will contact you by phone or email to make further arrangements in relation to your future appointments. We highly recommend that you follow directions of your health-care provider in relation to your symptoms.
